Arranging For Your Discharge
 Your
doctor will determine when you are ready to go home, and
will order your discharge. You will be given instructions
about follow-up care, medications, etc., before you leave.
Please make arrangements for your transportation home so
your discharge will not be unnecessarily delayed.
A Continuum Of Care
In preparation for your discharge, Albemarle Hospital can
arrange a variety of care and services for you. Just call
384-4580 - our discharge planning staff will be happy to
show you how to secure home care, home health aides, and
a variety of other community health services, including
how to enter a nursing home or a similar facility if required.
