Albemarle Hospital respects your rights and recognized
that each patient is an individual with unique healthcare
needs. Because of the importance of respecting each patient's
personal dignity, care is given in a considerate and respectful
manner and is focused upon each individual or child/adolescent's
needs.
Patient Rights - Adults
- The right to care that is considerate and respectful
with recognition of personal dignity and individuality.
- The right, within the law, to personal privacy and confidentiality
of information and to receive care in a safe and secure
setting.
- The right to impartial access to treatment or accommodations
that are available or medically indicated regardless of
race, creed, sex, national origin or sources of payment
for care.
- The right to have your pain recognized and addressed
appropriately.
- The right to know the name and professional qualifications
of the individuals caring for you and to know who is primarily
responsible for your care.
- The right to obtain complete and current information
concerning your condition, treatment and informed consent
in order to promote total patient involvement.
- The right to access people outside of the hospital environment
by means of visitors and by verbal and written communication.
If you do not speak or understand English you should have
access to an interpreter whenever possible.
- The right to information and treatment alternatives
necessary to make medical ecisions that reflect your wishes.
- The right to know if the hospital has relationships
with outside parties that may influence your treatment
and care.
- The right to refuse to participate in any research
projects.
- .The right to refuse treatment, to the extent permitted
by law and to be informed of the medical consequences
of such refusal.
- The right to be transferred to another hospital. The
reason for transfer can be based on your request and/or
medical necessity. You will be transferred only after
receiving a complete explanation of and the alternatives
to the transfer. The transfer must be acceptable to the
receiving facility.
- If you are disabled, you have the right to access hospital
services in accordance with ADA guidelines.
- The right to be involved in ethical questions that may
come up during their care, to include, settling conflict,
withholding resuscitation, refusing or stopping of life
sustaining treatment and involvement in research studies
or clinical trials.
- You have the right to make advance directives (such
as living will, health care power of attorney and advance
instruction for mental health treatment) and to have those
directives followed to the extent permitted by law.
Patient Responsibilities - Adults
- You are responsible for providing accurate and complete
information about present complaints, past illnesses,
hospitalizations, medications, and other matters relating
to your health to ensure we are able to give you the best
are possible.
- The patient and family are responsible for reporting
perceived risks in their care and unexpected changes in
the patient's condition.
- .You are responsible for questioning, understanding,
and following, to the best of your ability, the treatment
plan recommended by your physician, nurses, and other
healthcare workers.
- You are responsible for your actions should you refuse
treatment.
- You are responsible for being respectful of the rights
and privacy of other patients and the hospital personnel
by assisting in the control of noise and by encouraging
the cooperation of your visitors to do the same.
- You are responsible for being respectful of the property
of others and of the hospital.
- You are responsible for ensuring that financial obligations
for your healthcare are fulfilled as promptly as possible.
- You are responsible for following hospital safety regulations
and other rules affecting patient care and conduct.
Patient Rights and Responsibilities - Children/Adolescents
In addition to the rights and responsibilities afforded
the adult patients, minor patients and their parents or
guardians have the following rights and responsibilities:
- To participate in the treatment process.
- To ensure that their child (children) follow the agreed
upon course of treatment.
- Providing informed consent for their minor child (children).
