The Heart Failure Clinic at Albemarle Hospital is an outpatient service that provides special treatment for those with Heart Failure.  The program is staffed by a Physician Assistant and a Nurse Educator who are on site.  We work closely with your Physician to provide the best individual treatment plan for you. 

We also provide education on not only Heart Failure (the disease), but also:

• your medications
• diet
• exercise
• signs and symptoms
• when to call your doctor or the Heart Failure Clinic

Our goal is to decrease your hospitalizations, and increase your quality of life. 

Ask your Physician for a referral to the Heart Failure Clinic at Albemarle Hospital if you have been diagnosed with Heart Failure.  Service is usually covered by insurance or Medicare.
